The documents checklist

Everything an attorney and the insurance companies will ask for, in one place. Check items off as you find them. Missing some? That's normal. An attorney can request much of it for you.

1. The police report

Report number and the responding agency (city police, county sheriff, or Georgia State Patrol) A copy of the full crash report, from the agency (many Georgia agencies offer reports online) Any citations issued, and to whom

2. The at-fault driver and vehicle

Driver's full name, phone, and address License plate, make, model, and color The vehicle owner, if it's not the driver Employer, if they were driving for work (a company truck, delivery, or rideshare)

3. Their insurance company

Insurance company name and policy number (from their card or the police report) Your claim number and the adjuster's name and phone Every letter or email the insurer sends you

Georgia's minimum liability coverage is often too low to pay for a serious injury. That's why your own UM coverage matters.

4. Your UM coverage and your own policy

Uninsured motorist (UM) coverage pays when the at-fault driver has no insurance, not enough insurance, or can't be found. Ask your agent for your declarations page.

Your declarations page (policy summary) UM limits, and whether it's add-on (stacks on the other driver's limits) or reduced-by (subtracts them) MedPay, collision, and rental coverage Policies of relatives you live with, since their UM may cover you too If you were a passenger: the UM on the vehicle you were riding in

5. Medical records and bills

A list of every provider: ambulance, ER, doctors, therapy, imaging, and pharmacy Visit dates, with no gaps you can't explain Itemized bills, plus the health insurance explanations of benefits (EOBs) showing what was paid Receipts for prescriptions, braces, and mileage to appointments

6. Lost income

Pay stubs from before the crash A letter from your employer confirming the missed days and your pay rate Doctor's work notes or restrictions

7. Evidence from the scene

Photos and video of the vehicles, the scene, road signs, and your injuries over time Witness names and phone numbers Dashcam footage, and nearby businesses that may have cameras (ask quickly, because footage gets erased) Repair estimates, total-loss letter, and rental receipts

Don't send these through a website form

Keep your Social Security number, full medical records, and driver's license images to yourself until you have signed with an attorney and they ask for them through a secure channel.
